To report a COVID-19 case at a long term care facility please:
Steps to report:
1. Fill out report form.
2. Fill out appropriate line lists.
a. ARI Resident Line List
b. ARI Staff Line List
3. Email both report form and line list(s) to: BC.Health.Public.LTCF@browncountywi.gov (or fax to (920)448-6449).
a. In the subject line, include facility name with suspect outbreak type (COVID-19).
Next steps:
- Expect an autogenerated email confirmation receipt that the line list was received.
- Staff will reach out if questions arise as we review your submissions.
- Please be sure to include updated contact information with your submission.
