Comprehensive Community Services (CCS)

Comprehensive Community Services (CCS) is a community based rehabilitation program that provides consumer-centered and directed psychosocial services that promote recovery. Services are provided to members to

• Better manage symptoms of their mental health and/or substance abuse conditions
• Increase independence
• Achieve effective levels of functioning in the community and at home

CCS is designed to empower adults and children to take an ACTIVE role in treatment and recovery and engage in positive relationships with staff and peers. As part of the program, CCS consumers will have a Recovery Team. The Recovery Team, identified by the consumer, participates in assessment, service planning and delivery, psychosocial rehabilitation, and the evaluation of desired outcomes.

Individuals Seeking Services Must:

• Be residents of Brown County
• Have a mental healh or substance use issue that requires treatment or intervention above outpatient care
• Be enrolled in Medicaid
• Be deemed eligable via the state-approved functional screen

• Service Facilitation (Case Management)
• Individual Skill Development
• Psychotherapy
• Substance Abuse Treatment
• Peer Support
• Medication Management
