How To Make A Referral

To make a referral to the Brown County Birth to 3 Program, contact the Birth to 3 unit using any of the following methods:

The following will be necessary when making a referral:

• Child's Name and Child's Date of Birth
• Address Where Child Resides
• Parent/Caregiver Name, Address and Phone Number
• Reason for Concern
• Name of Child's Current Physician

Referrals to Birth to 3 can be made by:

• Parents
• Physicians
• Child care providers
• Relatives
• Other Programs/Supports Working with Families with Young Children
