Complaint Information
We understand there can be concerns with services you receive that rise to the level of a grievance or complaint. For informal complaints, please contact the worker's supervisor or program manager. If that does not lead to a satisfactory resolution, you may file a formal complaint by completing and submitting a grievance form.
