PASSPORT INFORMATION

THE FOLLOWING INFORMATION IS REQUIRED:
You will need a current application form (Form DS-11) properly completed. Forms can be obtained from the County Clerk’s Office or by visiting the website at travel.state.gov. You can fill the form out and bring it in. Please do not sign the form until requested to do so in the presence of an acceptance agent. Forms must be filled out in black ink only.
Proof of Citizenship: A certified birth certificate must include your given name and surname, date and place of birth, full name of parents, date the birth record was filed, and the seal or other certification of the official custodian of such records. Naturalization and consular report of birth abroad are considered proof of citizenship.
Proof of Identity: A valid driver’s license or picture ID.
Photograph: A passport photo following the Passport Agency specifications. Your picture can be taken in the County Clerk’s Office for a fee of $17.00.
FEES FOR PASSPORT BOOK
U.S. Passport Book - (16 years of age or older) - $130.00
U.S. Passport Book - (15 years of age or younger) - $100.00
U.S. Passport Card - (16 years of age or older) - $30.00
U.S. Passport Card - (15 years of age or younger) - $15.00
A $35.00 per applicant processing fee, payable to the County Clerk’s Office in cash, check, or money order.
All applicants must appear in person. If the applicant is 15 years or younger, both parents are required to appear along with their proper identification. If applicant is 16 or 17 years old, one parent needs to be present with identification.
OFFICE HOURS
Applications for passports are processed Monday – Thursday, between the hours of 7:30 a.m. – 4:15 p.m.*** and Friday 7:30 a.m. - 10:45 a.m.***, at the Brown County Clerk’s Office, 305 E. Walnut St., Room 120.
*** Citizens must be present at the County Clerk’s Office well in advance of 4:15 p.m. (M-TH) or 10:45 a.m. (F) to ensure their needs are properly addressed and/or business conducted in a timely fashion.
