To begin a new Guardianship under Wis. Stats. Chapter 54 please complete the following documents:

Return the completed forms to the Register in Probate office. A judge will be assigned to the case, and a hearing date will be assigned.
You will be assessed a $500.00 deposit made payable to the Guardian ad Litem. This deposit may be waived if the petitioner is indigent or has a financial hardship, and the judge will determine fee responsibility at the time of hearing.
The petitioner must have the proposed ward personally served with a copy of the filed documents (listed above.) The process server must complete the Certificate of Service (form GN-3121) and file with the Register in Probate.
The petitioner must provide a copy of the filed documents (listed above) by mail or personal service upon the following individuals:
1. The proposed ward's counsel, if any.
2. The proposed ward's guardian ad litem.
3. Any presumptive adult heirs, as specified in s. 851.09, of the proposed ward.
4. Any other interested persons, unless specifically waived by the court.
5. The agent under any durable power of attorney or power of attorney for health care of the ward.
6. Any person who has legal or physical custody of the proposed ward.
7. Any public or private agency, charity, or foundation from which the proposed ward is receiving aid or assistance.
8. The proposed guardian for the proposed ward.
9. Any other person that the court requires.
After service is completed, the Affidavit of Service (GN-3120) must be completed and filed with the Register in Probate prior to the hearing.The following forms must be completed and brought to the scheduled court hearing:

The Guardianship Inventory click here to access form must be completed by the person appointed as guardian of estate 60 days after appointment, and the proper filing fee paid ($20.00 for assets valued under $50,000 - .2% of value of assets valued over $50,000.)
