To petition for the appointment of either a stand-by guardian or a successor guardian, please complete the following documents:

- Return the completed forms to the Register in Probate office. A judge will be assigned to the case, and a hearing date will be assigned.
- You will be assessed a $500.00 deposit made payable to the Guardian ad Litem. This deposit may be waived if the petitioner is indigent or has a financial hardship. At the hearing, the judge will determine fee responsibility.
- The petitioner must provide a copy of the filed documents by mail or personal service upon the ward and all interested persons.
- After service is completed, the Affidavit of Service (GN-3120) must be completed and filed with the Register in Probate prior to the hearing.
