Death Certificate
Statewide issuance of vital records allows any Register of Deeds office in the State of Wisconsin to issue Death certificates from all Wisconsin counties from September 1, 2013 to present.
The fee for vital records is $20.00 for the first copy and $3.00 for each additional copy.
There are three options available to obtain a vital record:
Apply in person:
All in-person requests are processed while you wait between the hours of 7:30 a.m. to 4:30 p.m. Monday through Thursday and 7:30 a.m. to 11:00 a.m. Fridays. Applicant must show valid identification with application.
Acceptable payment - cash, credit/debit (additional fees apply) and cashier's check or money order. Personal checks are not accepted.
Apply by mail:
Complete the Death application form (link below). All mail requests are processed the day they are received. Submit completed application, a photocopy valid identification and the correct fee.
