Domestic Partnership

The fee for vital records is $20.00 for the first copy and $3.00 for each additional copy.

There are three options available to obtain a vital record:
Apply in person:
All in-person requests are processed while you wait between the hours of 7:30 a.m. to 4:30 p.m. Monday through Thursday and 7:30 a.m. to 11:00 a.m. Fridays.  Applicant must show valid identification with application.
 
Acceptable payment - cash, credit/debit (additional fees apply) and cashier's check or money order. Personal checks are not accepted.

Apply by mail:
Complete the Domestic Partnership application form (link below).  All mail requests are processed the day they are received. Submit completed application, a photocopy valid identification and the correct fee.
