BadgerCare Plus
BadgerCare Plus is a health care coverage program for low-income Wisconsin residents including children, pregnant women and adults. Click here for further information about BadgerCare Plus including eligibility criteria.
Medicaid
Medicaid is a health insurance program for those who are age 65 or older, blind or disabled. There are financial and non-financial eligibility criteria for the program. Income and assets may be considered in the eligibility decision. Click here for further information about Medicaid.
Once enrolled in BadgerCare Plus or Medicaid,
you are issued an identification card from ForwardHealth
How To Apply
Online Application
Apply for BadgerCare Plus or Medicaid online by using the ACCESS website. The website is: https://access.wisconsin.gov. Click on the box that states "Apply for Benefits".

Call the Bay Lake Consortium at (888) 794-5747.
In Person Application
Come into the agency Monday through Thursday, 7:30am to 5:00pm and Friday, 7:30am to 11:30am. If you are only requesting the health care programs, you may pick up an application and return it to the agency. You'll need to complete some brief paperwork, and then meet with client registration staff who will set up an appointment for you with an eligibility worker.
Mailed Application
Request that application forms be mailed to you by calling the Bay Lake Consortium at (888) 794-5747. You can complete the applications and mail them back to the agency. An appointment will then be set up with you, if one is needed.
Health Insurance Marketplace
If you are not eligible for BadgerCare Plus or Medicaid, you may have other coverage options through the Health Insurance Marketplace.
