COMMUNICABLE DISEASE REPORTING

All reportable diseases (according to Wisconsin law DHS 145) require both a laboratory report AND a provider report to be sent to the public health agency in the jurisdiction where the patient resides. Provider reports for Brown County residents can be submitted via the Wisconsin Electronic Disease Surveillance System (WEDSS) OR through one of the following report forms via fax to (920) 448-6479:

*To report any confirmed OR suspect Category I Disease (active tuberculosis, measles, pertussis, hepatitis A, etc.) in Brown County, please call (920) 448-6400 and use option 4 to reach the intake nurse prior to submitting a report. Use the after-hours option if applicable.

Fax the appropriate form to (920) 448-6479 along with a copy of the chest X-ray/CT impression, lab results, and current medication list if possible.

If you have previously reported an LTBI case and the patient received treatment through private pharmacy/means other than BCPH, please send us the Latent TB Infection Follow-Up Report F-44125 (https://www.dhs.wisconsin.gov/forms/f4/f44125.pdf) upon completion of treatment.
